Details to collect about you from consultation form: Prior to your first treatment, you will be asked to complete a client consultation form, where you will share details of any medical condition, preferred treatment outcome, contact information, and environmental factors. This is to ensure that I provide you with an appropriate treatment. I will securely store this form in the locked cupboard.
